Joe Simpson Gravity Forms is fantastic. My only gripe is the recent over-styling of the plugin (like a lot of plugins) trying to add their own branding and end up looking odd in a WordPress site. GF has even added a custom font lately which just makes it all look a bit bizare, rather than just making a plugin which feels integrated into WordPress (like Woo does)
